I understand that, in accordance with the Centre’s policies, for missed classes, I am eligible for one credit per subject each month, and I must notify the Centre at least 24 hours in advance of the class to receive the credit. All other classes will be forfeited as we are reserving the spot for your child.*

I understand that the program uses positive reinforcement and clear expectations to guide student behavior. I acknowledge that if my child damages school property, I may be held financially responsible. I also understand that repeated disruptive or unsafe behavior may result in a parent meeting, temporary suspension, or removal from the program.*

I, the undersigned, give permission for my child to participate in the after-school programs. I confirm that I have read, understood and agree to the policies mentioned above.**
